I made this using the actual chords tom fletcher played on his ukelele when he did a youtube 
video cover of his own song, so i am 99% positive all of these are correct. 
I also was able to play along perfectly with music video. Two chords i couldn't find
 the name for so i will show them below and name them with two stars after note name
