Capo on 1 or Matt plays is down 1/2 step with capo on 2nd:

Verse: Bm - F#m - G - D

"When we were young and hit like hammers":
Chorus: G - F#/D - A/G

Then:
"And I know it's real slow, honey"
Bm-G-A/G

Listen to how he plays it acoustic and you can figure out the pull-offs/hammer-ons, but 
this is the general idea.
