I'm not sure what some of the chords are so here's a few notes:
(The Em/C chord is a C with the 2nd string played open;  I put Bm5 for the Bm chord 
at the 5th fret, and Eb3 for the Eb chord moved up to the 3rd fret;  Lastly play the D chord 
the first string open or play a G instead)
