If you play the F open or stay on the top 3 strings, or you can
skip the F and play a D-shape 3 frets up. then you can slide from 
there up another 2 frets for the last chord.  
* playing a D shape up 3frts higher = F in the 3rd position
and 5frts = G in the 3rd position. 
Playing it the second way sounds better, it's up to you
