Just play a "D" and pick the top "D" Because it is in drop "D"tuning then  
the "G" then strum the bottom 3 strings and on the second time you do the same  
but do the back strum and then hammer on the 4th fret with the little "e" 
then you hold this for another quick downbeat like a Dsus4
